Transitional Housing – Exchange House

Exchange House is a transitional, residential facility for adults 18 years and older, who are infected with HIV and are homeless or are at risk of becoming homeless. All residents must be fully ambulatory or in minimal need of assistance with activities of daily living. The Exchange House is designed to provide a means for homeless adults with HIV to reside in a stable environment and cultivate the resources and skills that will enable them to become self-sufficient and live independently within the community.

Exchange House is located in Houma, LA and serves individuals from Assumption, Lafourche, Terrebonne, St. Charles, St. James, St. John the Baptist, and St. Mary parishes. Exchange House operates 24 hours a day, 7 days a week.

For Client Eligibility one must:

  • Be a Louisiana resident
  • Be at least 18 years of age
  • Have a diagnosis of HIV/AIDS
  • Be currently homeless or at risk of becoming homeless
  • Be low Income
  • Have a clean drug screen
  • Be willing to participate in alcohol/substance abuse treatment if asked
  • Consent to random drug screening
  • Disclose mental health history
  • Agree to start/continue treatment for any current and/or ongoing mental disorder(s)
  • Consent to a background check
  • Have a current primary care physician
  • Be willing to live in a group environment
  • Agree to abide to house policies and rules of Exchange House
  • Be able to live independently and need only minimal assistance
