Permanent Supportive Housing

Permanent Supportive Housing (PSH) units are special rental apartments that come with supports for people who have difficulty living successfully in communities and may become homeless or institutionalized without the support.

PSH employs the Housing First model, which is a strategy to end homelessness that places individuals into stable, permanent housing as quickly as possible. Through intensive case management and links to community-based services, Housing First allows individuals to achieve the maximum level of self-sufficiency for which they are capable and helps them integrate into the community in which they live.

For Client Eligibility:

  1. A member of a household must have a substantial, long-term disability as determined by DHH including but not limited to any one of the following:
    • Serious Mental Illness
    • Addictive Disorder
    • Developmental Disability
    • Physical, sensory, or cognitive disability occurring after the age of 22
    • Disability caused by chronic illness
    • Age related disability
  2. Must be in need of housing support offered by the PSH Program. PSH provides housing supports to persons with disabilities including youth existing foster care, homeless persons and elders who need these supports to be able to live in the community and not become evicted or homeless.
  3. Must be extremely low income (earn less than 30% of the area’s median income). For Orleans Parish for a family of one that would be $12,550.00.

Steps for Obtaining a PSH Unit:

  1. Submit an application: You can submit an application through the Office of Aging and Adult Services. An application can be obtained by calling 504-568-3369. It is important that the application is returned by the due date. Applications are only accepted during specific time periods.
  2. Provide verification documents: At the end of the application period all applicants are placed in to a lottery. Those names that are placed on the top of the list will be contacted by the Tenant Selection Manager and asked to provide documents that verify the information provided in the application.
  3. Referral to property manager: Once all the necessary documents are received, the individual is matched to available units based on preferences and bedroom size needed as indicated on the application form. The Tenant Services Liaison (TSL) then assists the individual in making application to the property manager of the available unit.
  4. Move into the unit: Once the property manager has approved the individual’s application, the TSL and a Housing Support Team will assist the individual with understanding the terms of the lease and moving into the unit.
  5. Obtain Services: Once an individual is moved into a PSH unit, the Housing Support Team will work with the individual to develop an individualized service plan. The purpose of this plan is to assure that the services needed by the individual to maintain tenancy are provided.
